If you have visited or contacted M A R A wellbeing I Mary Rayner will retain your contact details on a password protected phone, Ipad and Microsoft outlook account, which could include your full name, contact number, email address, in order to discuss and confirm appointments.
An Initial Consultation is performed prior to your first treatment, you will need to supply contact information, Medical history, and be asked to give details on your lifestyle. The information requested in the consultation process is for the safety of the client & of M A R A wellbeing. Medical questions are mandatory to ensure the client is safe to receive treatment & the therapist is safe to give treatment. Contact details are required to ensure I can contact the client if an issue or error with treatment should arise. Lifestyle & Skin type questions are asked to gain understanding of the client & to give a better, more tailored treatment. These are not mandatory & you do not have give answers for these if you do not wish to. I may make notes during or after the consultation process, these are my clinical assessments, treatments given, recommendations & personal notes that I feel are useful to your treatment. Should you wish to see these please ask, they are also strictly confidential kept & stored in accordance with The Data Protection Act 1998 & the GDPR Act 2018. It is a requirement that the client, parent or guardian sign the consultation sheet to confirm consent to receive treatment from M A R A wellbeing, for M A R A wellbeing to retain their details, that the information given is true & correct to the best of your knowledge & to consent that M A R A wellbeing will not be held responsible for any injury or allergy that has occurred to the patient where a medical condition was not disclosed in the consultation process. You may be asked to sign a disclaimer to consent to treatment if you have any medical conditions you currently have that may affect your treatment. On subsequent appointments will be asked for an update of your medical condition(s), injuries & any medication you may be taking. Should your medical condition ever change you must make the Therapist aware of this, if the client has not been for some time they may be asked to complete a new consultation and/or disclaimer.
M A R A wellbeing reserves the right to refuse or terminate treatment at point if treatment is deemed unsafe or in appropriate.
I can ensure that all personal information supplied during the consultation process is strictly confidential & is kept & stored in accordance with The Data Protection Act 1998 & the GDPR Act 2018 & is never passed on to Third Parties. The information taken in the consultation process is only viewed by myself and the client and is kept only on paper in a locked cabinet, Excluding your full name, contact number(s) and email address which are stored on a password protected phone, Ipad & or Microsoft outlook and Mailchimp software accounts. Should you no longer wish to attend M A R A wellbeing and wish for your details to be destroyed please contact me directly by email at firstname.lastname@example.org or by phone on 07922 696403.

You are able to contact me via my website, designed by Health Hosts, through a contact form, the form collects and stores your submitted data, your name, email address along with your message to enable me to communicate with you and answer and questions you may have. You will be required to tick a box to consent to your data being submitted and stored. Microsoft Outlook, Mailchimp and Health Hosts are fully compliant to the New the GDPR Act 2018 and the existing Data Protection Act 1998.
